A. Hypokalemia, thiamine deficiency, hyperthyroidism are the more common causes. Subacute or chronic organophosphate toxicity, myasthenia gravis, polyneuropathy (ie diabetes mellitus), polymyositis, hypocalcemia, hepatic encephalopathy (ie from hepatic lipidosis), and muscular dystrophy and also possible but less common causes.
